Medium health is a healing item that, when picked up, grants the player 25 health points. It provides enough health that the player can withstand an extra hit or two, depending on how much armor the player has and the difficulty level being played, which is useful without overpowering the player.

In most of its appearances, medium health is one of the most common healing items found throughout the games, and occasionally appears in secrets, usually with more than one in them.

- In Serious Sam 1, Serious Sam: Next Encounter, Serious Sam HD and Serious Sam: Double D, it is a floating vial of green liquid.
- In Serious Sam Advance, it is a vial of blue liquid.
- In Serious Sam 2, it is a floating red cross with a semi-transparent white circle surrounding it.
- In Serious Sam 3: BFE, it is a pack with a grey camoflage pattern and a red cross on its upper side.
- In Serious Sam's Bogus Detour, it it is a floating circular-shaped object with a red cross on it.
